1) Our Responsibilities
- We are required by law to maintain the privacy and security of your protected health information (PHI).
- We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
- We must follow the duties and privacy practices described in this Notice and give you a copy of it.
- This Notice applies to services provided through Prime Doctor RX.
2) How We May Use and Disclose Your PHI
The law allows (and sometimes requires) us to use and disclose your PHI in certain ways. We may use and share your PHI for:
- Treatment — to provide, coordinate, or manage your care (including with other clinicians, labs, or pharmacies as needed).
- Payment — for billing, claims, payment processing, collections, and related activities.
- Healthcare Operations — to run our practice, improve quality, train staff, audit, evaluate performance, and support compliance and administration.
3) Other Uses and Disclosures We May Make Without Your Authorization
We may also use or disclose PHI, as permitted by law, for purposes such as:

- Public health and safety activities (for example, preventing or controlling disease, reporting adverse events, or certain product recalls).

4) Your Rights
You have the following rights regarding your health information:
- Get a copy of your medical record (paper or electronic) in most cases.
- Ask us to correct your medical record if you believe it is incorrect or incomplete.
- Request confidential communications (for example, asking us to contact you at a specific email address).
- Ask us to limit what we use or share (we are not required to agree in all cases).
- Get a list of certain disclosures we have made of your PHI.
- Get a copy of this Notice at any time.
- Choose someone to act for you (such as a medical power of attorney), as permitted by law.
- File a complaint if you believe your privacy rights have been violated.
5) Complaints
If you believe your privacy rights have been violated, you may file a complaint with us by emailing support@primedoctorrx.com.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights (OCR). We will not retaliate against you for filing a complaint.

7) Changes to This Notice
We may change the terms of this Notice, and the changes will apply to all PHI we maintain. If we make material changes, we will post the updated Notice on this website and update the effective date.
This Notice is provided as part of Prime Doctor RX’s privacy practices and applies to patient information handled in connection with services we provide, consistent with applicable federal and Texas law, and other applicable state laws where care is provided.
