6) Labs, Prescriptions, and Follow-Up
Lab testing may be required before starting treatment and periodically thereafter. Prescriptions and treatment plans are provided only when a licensed clinician determines they are clinically appropriate and permitted by applicable law. There is no guarantee that a prescription will be provided.
7) Your Responsibilities
- Provide complete and accurate health information
- Complete required labs and follow-ups in the requested timeframe
- Report side effects, adverse reactions, and meaningful symptom changes promptly
- Use a private environment and do not record sessions without permission

By using Prime Doctor RX telehealth services, you acknowledge you understand the information above, had the opportunity to ask questions, and consent to receive care via telehealth. You may withdraw consent at any time by discontinuing Services; however, withdrawal may limit our ability to provide ongoing care remotely.
